How to become a US Chess affiliate?

Hi, I am a club TD and am wanting to run my first tournament.  However, I am a bit confused about the US Chess affiliate program.  How do you become one, can you just be an affiliate by yourself, where is the forum to apply, ect.

Rule 23A states "Tournaments rated by US Chess must be organized by a US Chess affiliate or by US Chess itself."

To run tournaments, you need a certified TD and an affiliate to run the events through. Affiliates can be started by paying the yearly affiliate fee. The affiliate then can add authorized tournament directors.
