One simple setup i like against b6 is:
What to do against 1. e4 g6/b6

Against the modern, I believe in an English Attack like setup, with d4/e4, Nc3, f3, Be3, and Qd2. Easy to play for mate if black doesn't play carefully, and a good place to begin a middle game even if he does. Keeps early game thought to a minimum, if you use similar against Pirc, Sicilians, Philidor, etc.
