Not always. There once was a guy cursing in chats from many accounts with the same name but different indexes (like xxx20, xxx21 etc). One day the chess.com staff decided to ban all xxx accounts. The problem is that not all xxx accounts belonged to that abuser and at least a couple of innocent players who also had names similar to xxx were banned as well.

It’s mostly true, but in some cases the club owners are actually forced to cooperate. If they don’t, the attacker mass report them using alt accs or friends, and they end up getting banned. So yeah, it’s often due to negligence, but sometimes they’re just stuck and can’t do much about it.

I wouldn't be so dismissive.

Up until relatively recently such an attack may have been possible because of this: https://www.chess.com/clubs/forum/view/muting-of-admins

It's been fixed now though.

Although I understand even back then it only resulted in people getting temporarily muted, it wasn't a permenant problem; apart from getting some scary-sounding automated messages.
