Can someone explain why a draw from repetition did not occur?


Yes, Yes I know I missed mate 3 times, and forgot to take their queen; this potentially could be my worst game but why wouldn't it draw?


Exactly the same position has to be on the board 3 times in order for the game to be a draw. So just you shuffling moves back and forth will not do.

+1 except for the last part. I don't think this is automatic in chess.com live games - a player still must send a draw offer. If sent then it is automatically accepted, but the player must send a draw or the game keeps going - I believe a position repeated 5 times in live chess is an automatic draw (not as sure on this one though).

It's also an automatic draw, but the rule is 50 consecutive non-pawn moves. As soon as a pawn moves, the counter resets.

To make it even more confusing, it doesn't have to be a repeat of the board in consecutive moves either. I was in a position where I was "walking my opponents king" across the board and think of it like a pendulum. It started with the base position and I walked him 3 moves one way and he squeaked out and walked 3 moves back to the same position (so 2nd time the board was exactly the same) then I walked him in the opposite direction and he immediately bounced back to the original position for 3rd identical board position and to be blunt, he walked me from a win into a stalemate in 8 moves. It took me a while to grasp that it was a three-peat of the same board position and if it was an OTB game it likely wouldn't have been noticed.

Yes all draws by threefold or 50 move are automatic, you don't have to claim.

both players need to play the same move 3 times for a draw

To be clear its not repetition of moves, its repetition of position!
FIDE Artical 9.2
The game is drawn upon a correct claim by the player having the move, when the same position, for at least the third time (not necessarily by a repetition of moves):
a. is about to appear, if he first writes his move on his scoresheet and declares to the arbiter his intention to make this move, or
b. has just appeared, and the player claiming the draw has the move.
Positions as in (a) and (b) are considered the same, if the same player has the move, pieces of the same kind and colour occupy the same squares, and the possible moves of all the pieces of both players are the same.Positions are not the same if a pawn that could have been captured en passant can no longer be captured in this manner. When a king or a rook is forced to move, it will lose its castling rights, if any, only after it is moved.
Note: Obviously chess.com handles this, and the 50 move rule, differently to OTB play and the draw is automatically given. Assuming you are making some form of calculation with your moves it is very easy to keep track of and to recognise repetitions of a position when playing 30 minute Rapid or longer time controls.
