Move 12 isn't actually all that bad - and you ended a piece up after white's blunder 15. Bd5
18. ...a5 is a mistake, as it allows white to win a pawn back (at worst), and Qe6 just sets up a fork.
29. ...h5 was a mistake - d5 hardens your position and stops the queen attacking the b7 knight. Not a disaster though and still an even position
32. ...Qe6 is a massive blunder, though I'm guessing you were short of time at this stage, so excusable. Leaving your queen where it was and playing virtually anything else is better (you didn't need to be so worried about 33. e6)
