Where did my attack go wrong?

This was a game played against a considerably higher rated opponent than myself, so I thought I did alright. But it did end in my utter destruction. However, towards the end, I thought I had a really good attack going on the King, but I failed. Was my attack doomed from the start, or could it have succeeded if I played the correct moves?

I actually didn't see an "attack" on the Black King. Could you point it out?

I suppose I meant a build-up to an attack. I mean, maybe there wasn't even a chance of attack? I don't know. I apologize for my ignorance, as you can tell by my rating I'm still quite new to chess. I was putting pressure on the right side of the board in hopes of bringing a rook up there for a back rank mate.

Please don't apologize. I saw that you tried some "build-up" for a counter attack. But you gave up so many pawns, that your own pieces and King were continually open to the type of tactics your opponent used. Such as the rook+knight trade for the Queen(which actually turned out worse because you didn't immediately take the Knight with your rook, allowing your opponent to take your Bishop making it a rook for queen trade). It's difficult to start a counter-attack if you have given up the pieces that would have made your king safe to give you time for a build-up. This is merely my tiny opinionSmile

The game was lost earlier with 16.Bb6? which allowed Black the double attack of 16...Qh4 threatening both mate and the unguarded N on a4 and eventually leaving White down three pawns.  Always be aware of unguarded pieces and unguarded (or underguarded) squares near the king (yours and your opponents) since they are frequently the basis for tactics.  

Later, 25.b4 would have been better than White's actual 25.g4? because b4 keeps Black's rook out of the 2nd rank.  Next, 26.Rxh5? misses the fact that White's queen is unguarded after 26...Rxc2+.  26.Qh3 would have been much better and kept alive some hopes for an attack on Black's king.  After these two moves there was never much hope for an attack on Black's king.
