I have been doing 5 puzzles a day, but when I do puzzles, I *know* that there is a tactic involved. This is not true in a real game

I looked up your last few games. Both positional and tactical mistakes were made.
I don't see a specific way to improve for you. You can improve from a lot of angles. I don't recommend deep opening or endgame training. It's better for you to just know principles there.
Especially for blitz it's not needed so much. But once you are stronger definitely recommended.
As mentioned before tactics are the way to go. Let it be a mix from easy to hard, timed to unlimited. The more tactics you do the more you will see them in your own games. Chess.com doesn't allow you unlimited access to puzzle training without a premium membership. Maybe you could consider getting one. There are also other websites or apps to do puzzles.
Also analyse your games with the computer. I think the self-analysis is free. And then just try to understand what the engine is wanting. It will show you tactics and positional concepts. Although it's not easy to convert the information, your brain will soak it up and possibly you can apply it in another game.
Also try playing other time formats. Rapid for your calculation, Bullet for your instincts.

Puzzle rating is naturally higher than real rating. Take mine: 3000+ rating but im only 2100 blitz
For the past 12+ months, my blitz rating has been consistently oscillating between 1150 and 1350. I know the fact that my account was closed for several months probably plays a role, however, the pattern had started well before I closed my account.
If the breaks are excluded, my rating graph might as well be a sinusoidal function.
How can I break out of this cycle?
