How do you avoid blundering under time pressure?

Played 10 games of OTB rapid, with 10s increment. I ruined 5 winning games.

First allowed my opponents pieces to scoop up my extra pawns (was a draw). 

Then,l lost an endgame where I was up an exchange.

Next game, I hung a piece.

Next, I repeated 3 times accidentally. Final game, blundered mate in 2.

How do I stop this? Should I do more tactics and play more?

Avoid time pressure.

I say it with a great deal of sympathy for your desire to play correctly, having lost many times from the blunders of time pressure.

Manage your clock better.  Getting into a winning position is always a lot easier if you use more time than you can afford, and more time than your opponent does, to get there.

 Fair, but it's not even like I was spending masses more time than my opponent. More like a minute, and usually it's in mutual time trouble too that I blundered in

Blitz can help. If play 5 minute chess you will get use to playing fast enough that a 10 second increment will be enough to play on, in a complicated position you won't always convert a pawn lead but:

-You'll win more often because you'll get used to playing endgames fast.

-You'll hang less pieces in limited time, because that's how people improve in blitz.

Blitz rarely helps people's OTB game but hen you have problems with time trouble it really can.
