You really gotta need to start playing with a plan (any plan is better than no plan), stop reacting to your opponent's plan and start following through with your own, stop with mindless king attacks, and pay closer attention to the needs of the position, the various imbalances ... you can't just do what you want when you want to, you have to do what the position calls for. Okay, if I can somehow stretch that to 450 pages, I will be a millionaire.
There are situations where you need to react to your opponent's plan,prophylaxis ala Karpov. There are situations where you can ignore your opponent's plan. It is the experience, studying and playing that will guide you on what rule to follow.
