Chat Request vs Game Chat

Interesting, I think Chat Request is mainly for when people PM you? Game Chat is obviously during the game, but I haven’t really fiddled with those settings.
How do these features work? If I have Chat Request set to Friends Only and Game Chat set to Everyone, I'll sometimes get requests to accept or decline chat in-game. Other times, people that I'm not friends with will start chatting at any time without me giving permission. I want to leave the option to chat open and not disable it completely, but always get asked for permission first.
