Medics: what is the answer to this question?

There’s no harm in giving a shot, we have doctors also in forums, someone will likely be able to answer based on years of experience and it will be nice to have medical discussions also
It is recommended that women avoid pregnancy while on treatment for TB. The effect of the oral contraceptive pill is reduced by Rifampicin, making this method of contraception unreliable. It is recommended that women on treatment for TB use an alternative method of contraception.23 Sept 2015

Thanks, that’s a contraindication since you said it’s reduces the efficacy of Rifampicin Hmm and if that be an indication concerning women which i don’t think is... why so ? Will it be transmitted to the baby? I thought tb is gotten through inhalation of the droplets or skin penetration or ingestion only.

Okay. Tuberculosis is an infection caused by a bacteria called Mycobacterium tuberculosis mainly.
The drugs for the treatment of tuberculosis;
A) we have the FIRST line drugs which are:
1. Rifampicin
2. Pyrazinamide
3. Isoniazid
4. Ethambutol
5. Streptomycin
B)The SECOND line drugs or reserve drugs which are:
1. Ethionamide/prothionamide
2. Ofloxacin/levofloxacin/moxifloxacin
3. Cycloserine/terizidone
4.Kanamycin/amikacin
5.Capreomycin
6.Para-amino salicylic acid
C) and in some cases corticosteroids are used like prednisone ( anti-inflammatory
MECHANISM OF ACTION these drugs simply is to inhibit the production of mycolic acid which is a component of the bacteria that causes Tb. Mycolic acid prevents our white blood cells from fighting it and destroys the power house of our body cells ( mitochondria) . It also helps the bacteria to let’s say grow or multiply.

COCPS are combined oral contraceptive pills it is a combination of progestin and estrogen.
The two commonly used estrogens are ethinyl estradiol and mestranol which If you use cocps you will see.
While Progestins commonly used are levonorgesterel, norgestrel, norethindrone , norethindrone acetate and ethinodiol.
MECHANISM OF AACTION OF COCPS: So basically the way Cocps work is to inhibit ovulation by acting on certain hormones in the pituitary gland and also modifying the cervical mucus to make it difficult for sperm to travel simply

So, roommate asked me this question that a consultant asked when Combined oral contraceptives are used in the treatment of tuberculosis ( indication ) and without giving an answer left it for research . She couldn’t find the answer after searching the internet, she asked me and I was speechless and I’m also curious as I don’t see the correlation. Hence, I decided to turn this into a topic. As you can see even from the mode of action as I highlighted above of these two drugs; one is for killing a bacteria and the latter for preventing conception. How do they work together ?

But can tuberculosis be spread from mother to foetus ? Has it ever been reported or happened ?

Ok so you are saying cocps are given to protect the transmission of tb from mother to child by preventing conception, thanks doc 😉👍🏼

I was about to say this, thanks for saving me the stress
The passage I quoted says pregnancy is discouraged if you have TB.
I assume Rifampicin is used to treat TB(?).
Because this drug reduces the effectiveness of oral contraceptives you could end up with a pregnancy afterall.
So use another form of contraception if you are being treated for TB.
And yes, this is only relevant for women.
