For quite a while now (I strongly suspect months, certainly weeks at least), Game Review has been telling me things like "This is the Schloopdorp Reverse Anti-Orfendor opening, which you've played 0 times as white with a 0% win rate" when I know full well that I've played the Schloopdorp Reverse Anti-Orfender opening.
The "0 times at a 0% win rate" things makes my math pedantry nerve twitch, but it's not the important part, and I want to be clear that the real important part is that it's stuck. And not just at zero - for example, for one specific opening, I've noticed that it tells me "78 times as white with a 58% win rate" -- literally 78, literally 58% -- every single time that I play that opening.
In case it matters, I play the vast majority of my games as daily, and I know these observations to be true only for reviews of daily games. I do not know whether or not the same sort of issue occurs in any other format.
